QuizStar Subscription Reimbursement Request

Beginning November 1, 2009, subscriptions with six months or more remaining (expiration date of 4/1/2010 or later) are eligible to receive a prorated refund upon submission of the form below. If you choose not to request a refund, the remaining amount will be used only to maintain and update QuizStar. Refund requests must be submitted before December 18th to be processed. All refunds will be issued as checks.

Group subscriptions will be prorated in the same way.

The Transaction ID can be found on your payment receipt. To view your receipt, log in to your QuizStar account and click the "Account" tab. Click the link that says "receipt" for the payment for which you need reimbursement. The Transaction ID is in the upper right corner.
